​​Enrolment process

To enrol your child at Murphy's Creek State School, download the forms below and contact us on (07) 4630 5167 or admin@murphyscreekss.eq.edu.au to request an enrolment interview. Please bring the following completed forms for each child to your enrolment interview. Proof of date of birth (e.g. birth certificate) will be required for enrolment.

If you cannot download the documents from this page or you would prefer to have an enrolment package posted to you, please contact us. 

In most circumstances, we defer enrolments for at least one day to allow for all necessary arrangements for the best placement of your child to be made. This could take longer for children who may have special educational needs. 

A child enrolling from interstate will be located into a year level as close as compatible to their interstate schooling experience, ability and age. The process will occur as part of the enrolment process.
